After you schedule your appointment, you will be emailed a set of electronic intake forms. These forms must be completed prior to your appointment, otherwise your exam may be cancelled and a fee assessed. See our cancellation policy for more information. You will also need to upload a copy of your medical records as well as your Florida drivers license or State ID card. Please be sure to bring your drivers license or State ID card with you to your appointment. While it is not necessary, we do strongly recommend that you also bring a physical copy of your medical records with you to your appointment. Please note, we cannot accept records that have been uploaded to a disk or a USB flash drive.
During your Medical Medicinal Evaluation, your physician will review your records based on your electronic intake forms and can answer any questions you may have. If your physician approves you for a recommendation, he or she will then enter you into the Florida Medical Medicinal Cards Use Registry after assigning you with a Patient ID Number. Once you are entered into the Registry, the Florida Department of Health Office of Medical Marijuana Use (OMMU) will send you an email with step-by-step instructions on how to complete your application and pay the $75 state-required annual fee. On the OMMU website you can find a list of Instructional Guides to help make the process go smoothly from start to finish.
The application approval process typically takes 7-21 business days. Once your application and payment have been successfully processed by the OMMU, you will receive an email with your temporary Medical Medicinal Card. This temporary card can be printed out and will allow you to make your first purchase at a Florida medical Medicinal dispensary. What Conditions Qualify for a Medical Medicinal Card in Fort Lauderdale?
