Once your in-person exam has been scheduled you will receive two emails from our office: one confirming your exam date and time and another containing your electronic intake forms. Your intake forms are required to be completed at least 24 hours before your exam. If you fail to complete your forms prior to arriving for your exam, it may be cancelled and a fee assessed to your account. If you need to cancel or reschedule your exam for any reason, you may do so through our online Patient Portal. Please make sure you review our Billing Policy prior to cancelling or rescheduling your exam.
Please note, all patients are required to provide medical records and a Florida drivers license or State ID card. If you do not have your medical records on hand and are unable to obtain them, our records department can request them for you. Medical records uploaded to a disc or USB flash drive cannot be accepted. You must bring your Florida drivers license or State ID card with you to your appointment.
During your in-person exam, the Medical Assistant will conduct a basic physical examination to measure important vital signs—temperature, blood pressure, and heart rate. Your physician will then review your medical and your electronic intake forms, and will be happy to answer any questions you may have. If your physician determines that you qualify for a medical marijuana recommendation, you will be assigned a unique Patient ID Number and will then be entered into the Florida Medical Marijuana Use Registry. Shortly after being entered into the Registry, you will be sent an email from the Office of Medical Marijuana Use (OMMU) containing step-by-step instructions on how to complete the application and submit the $75 annual state-required fee for your Registry ID Card. If you experience any issues when completing your application, please contact the OMMU by phone at (800) 808.9580 or by email at medicalmarijuanause@flhealth.gov.
Within 7-21 business days of submitting your application and payment, you will receive an approval email from the OMMU containing your temporary Registry ID Card. This temporary card can be printed out and used to purchase medical marijuana products at any dispensary in Florida until your physical card arrives in the mail.
What Conditions Qualify for a Medical Marijuana Card in Miami Gardens?
In order to be eligible for a medical marijuana recommendation in Florida, you must be a permanent or temporary Florida resident and have a diagnosis of a state-approved qualifying condition such as anxiety, PTSD, or chronic pain, just to name a few. Temporary residents are able to use any TWO of the following documents to prove temporary Florida residency:
Mortgage statement or deed
Utility work order
Utility bill
Mail from any financial institution
Mail from any government agency
Proof of a Florida residential address combined with a supporting statement from the person with whom you’re residing
