Please note, payment is due at time of booking. More information about our affordable, all-inclusive pricing options can be found on our website. After your appointment is scheduled, you can expect an email from our office containing a set of electronic intake forms. In order to attend your exam, intake forms must be completed at least 24 hours prior to arriving at our office. We do require that you submit a copy of your most recent medical records. You can upload your records to your intake forms or bring a physical copy with you to your appointment. If you do not have your medical records on hand and are unable to access them, our records department will happily request them for you, free of charge. You will also need to provide a copy of your Florida drivers license or State ID card. Should you need to cancel or reschedule your appointment for any reason, you can do so online through the DocMJ Patient Portal. Please be sure to review our Billing Policy prior to cancelling or rescheduling your appointment.
During your appointment, your doctor will go over your medical records, intake forms, and will be happy to answer any questions relating to your condition and medical marijuana. You will find out during your appointment whether or not you are approved for a recommendation. If you are approved, your physician will enter you into the Florida Medical Marijuana Use Registry and you will then be able to apply for your Registry ID Card through the Florida Department of Health’s Office of Medical Marijuana Use (OMMU). Along with your application you will need to submit a $75 state-required fee. DocMJ staff is unable to provide any information or updates on applications.
Get Approval Email & Visit a Dispensary
Within 7-21 business days of submitting your application and payment to the Office of Medical Marijuana Use, you will receive an approval email letting you know that your application has been approved. The approval email will contain a temporary Registry ID Card that can be used to purchase medical marijuana products from any Florida MMTC (Medical Marijuana Treatment Center) until your physical card arrives in the mail. Should there be any issues with your application, the OMMU will notify you at the email address used to complete your application.
Part-Time Residents & Snowbirds Qualify for Florida Medical Marijuana
If you’re a snowbird or part-time Florida resident you may still be eligible to receive a Florida medical marijuana recommendation! Any 2 of the following documents may be used to prove temporary residency:
Mortgage statement or deed
Utility work order
Utility bill
Mail from any financial institution
Mail from any government agency
Proof of a Florida residential address combined with a supporting statement from the person with whom you’re residing
Other documentation that reflects proof of a Florida residential address
