If you are unable to reach us over the phone, please leave a voice message with your name, phone number and the reason for your call, and a Patient Care Coordinator will return your call within 24 hours.

Once your appointment is scheduled, you will receive a confirmation email as well as an email containing your electronic intake forms. It is important that these forms are completed AT LEAST 24 hours prior to your appointment. If your intake forms are not completed prior to your appointment, your appointment may be cancelled and a $50 fee assessed to your account. Please see our cancellation policy for more information.

All patients are required to provide medical records as well as a Florida drivers license or State ID card. Please be sure to bring your drivers license or State ID card with you to your appointment. Bringing a physical copy of your medical records to your appointment is not required but is strongly recommended. Please note, medical records that have been uploaded to a disk or a USB flash drive cannot be accepted. Don’t have access to your records? That’s not a problem! Our office can request records for you.

During your appointment, your Florida medical marijuana doctor will review your intake forms, go over your medical history and answer any questions you might have. If you are approved for a medical marijuana recommendation by your doctor, you will then be assigned a Patient ID Number and entered into the Florida Medical Marijuana Use Registry. Upon being entered into the Registry, the Florida Department of Health Office of Medical Marijuana Use, or OMMU for short, will email you instructions on how to complete the application and submit your state-required $75 yearly fee. If you experience any issues while completing your application or submitting your payment, please contact the OMMU directly at (800) 808.9580 or [email protected]. Please allow 7-21 business days for your application and payment to be processed. Once your application is approved, the OMMU will send you an email with a temporary card that can be used to purchase products immediately from any Florida dispensary.

 

What Conditions Qualify for a Medical Marijuana Card in West Palm Beach?

 

In order to be eligible for a medical marijuana card in the state of Florida, you must be a permanent or temporary resident with a diagnosis of a qualifying condition. Temporary Florida residents may use any two of the following documents to prove temporary residency: 

 

A mortgage statement or deed

A utility work order

A utility bill

A piece of mail from any financial institution

A piece of mail from any government agency

Proof of a Florida residential address combined with a supporting statement from the person with whom you’re residing

Any other documentation that reflects proof of a Florida residential address
