First Step: Fill out our pre-qualification questionnaire

We gather information from you to help determine whether your medical condition or conditions fall under the state’s approved qualifying conditions list. If you meet the criteria for a medical marijuana card, you can book an appointment with one of our medical marijuana doctors through our website. We have over 40 doctors on staff serving more than 30 locations throughout Florida and Ohio, so you can find compassionate care in a convenient location.

Second Step: Book your appointment and fill out intake paperwork

Unless you are booking for a pediatric patient, you can book your appointment on our website. Pediatric patients must be scheduled via phone. After you have booked your appointment, you will be emailed a link for your intake paperwork. You must complete this paperwork at least 24 hours prior to your visit. Your appointment can still be rescheduled if you have not completed the intake paperwork beforehand. 

Third Step: Meet with one of our medical marijuana doctors

At your appointment, our approved medical marijuana doctor will review your records and discuss your qualifying conditions. If you are a suitable candidate for medical marijuana use based upon the state’s standards and the doctor’s review, your information will be added to the state registry directly by the doctor. After, you will receive a link via email with the opportunity to apply for your medical marijuana card and pay your fee. Once approved, you will be provided with a unique registry ID number which serves as your temporary MMJ card until your annual card arrives in the mail. You generally have to wait 7-10 days for the actual card to come. 

Fourth Step: Locate a proper dispensary and experience relief

After you have received your card, our team will be glad to guide you to a dispensary to get your medical marijuana.
