Please note, our office is located on the 14th floor of the Superior Building. There is a paid parking garage (ABM Rockwell Garage) conveniently located behind the building next to Dunkin’ Donuts. The physical address for the parking garage is 801 Rockwell Ave, Cleveland OH, 44114. You may enter the Superior Building through the backdoor near the Phở Thắng Café. Parking rates are as follows: Each 15 Min: $2.50, Sat-Sun: $5, Daily Max: $10. Accepts: Amex, Bills, Checks, Coins, Debit card, Discover, MC/Visa.
Once your appointment is scheduled you will be sent a set of electronic intake forms via email. These forms must be completed at least 24 hours prior to your appointment date/time. You will be required to upload a copy of your Ohio drivers license or State ID card and a copy of your most recent medical records. If you do not have access to your medical records and are unable to obtain them, please let our office know so that our records department can request them for you.
Receive Approval & Visit a Dispensary
You will find out from your physician during your exam whether or not you have been approved for a medical marijuana recommendation. If you are approved you will be entered into the OHMM Registry. Next, you will need to complete your application and pay the $50 state-required fee for your Ohio medical marijuana card. This can be done at our office. As soon as your payment is processed (5 minutes or less) you will receive an email containing a digital copy of your Ohio medical marijuana card. Your digital card can be printed out or simply saved to your phone and is active immediately for purchasing products from any licensed dispensary in Ohio.
